Pathways Hospice provides hospice services that are intended to meet the physical psychosocial, practical, and spiritual needs of hospice patients and families/caregiver. Hospice services will be provided to terminally ill patients in their place of residence, which could be in the patient’s own private residence, a skilled nursing facility, adult foster care, or other living arrangements.
An interdisciplinary group of professionals and volunteers will develop with each patient and family/caregiver a plan of care, which will include, as appropriate, the following services:
How do I know if I quality for Hospice?
Many illnesses qualify for the hospice benefit including, but not limited to: Cancer, Heart and Lung Disease, Alzheimer’s or Dementia, Parkinson’s Disease, Stroke, Kidney Disease, Liver Disease, Multiple Sclerosis and Failure t Thrive or Debility. Many times a combination of diagnosis can also qualify a person.

What is the cost of Hospice Services?
Paying for Hospice has been made simple. 100% of the cost is covered by Medicare, Medicaid and most private insurances. Pathways Hospice services will be provided regardless of ability to pay.
